I'm posting this request on behalf of a new friend, Dennis Langer, who works
for a private firm and doesn't have access to the Internet. He is the first
and only librarian at his firm, and is coming up for performance review soon.
He'd like to know if he could get samples of the performance evaluation
forms used elsewhere, and also some sense of how other places handle the
review process -- e.g., who gets to fill out the forms? If you can help,
